At Melogada Health Center, located in a remote area of South Ethiopia’s region, a harrowing story unfolded that powerfully illustrates the stakes of primary care equity. A mother in labor, in a facility staffed by dedicated professionals, faced complications that could not be resolved onsite; a referral was triggered and the journey to the nearest hospital, navigating muddy terrain, rain and darkness became a race against time. The ambulance got stuck in the mud; the driver exited and pulled by hand as the mother screamed inside and after hours of effort, she survived; tragically, her baby did not. This event underscores the urgent need to ensure that essential obstetric services are available and resilient at the primary care level. We honor the commitment and achievements in improving access and equity of the MoH regional health bureaus, health workers and drivers who serve in challenging settings and reaffirm ACSIS’s dedication to bridging the gap between facility limitations and life-saving access.
